GENERAL RESPONSIBILITIES

Provides professional nursing care in a community setting, involving home visits, following established standards and practices while supporting and contributing to the mission, philosophy, policies, and objectives of the Stockbridge-Munsee Health & Wellness Center.  The Community Health Registered Nurse will work under the supervision of the Community Health Nurse for the Well and Frail Elder.

GENERAL RESPONSIBILITIES

All employees of Stockbridge Munsee Community must meet the following qualification:

  1. Must submit to and pass a pre-employment drug and health screening.
  2. Must be at least 18 years of age, unless otherwise stated on job description
  3. Must maintain an acceptable departmental attendance record
  4. Must be able to work weekends, nights and holidays
  5. Must obtain and maintain Elder/Youth License as stated on job description.

DUTIES

  1. Conducts initial client assessment to obtain thorough physical, social and environmental data.
  2. Utilize the nursing process to develop patient plan of care in accordance with physician orders and patient needs.
  3. Provides direct patient care and adjusts nursing care processes as needed. Identifies problems, modifies nursing care plan, and evaluates outcomes within an appropriate time frame.
  4. Administers prescribed medications and treatments, which may include dressing changes, lab draws, injections, according to approved Standards of Practice.
  5. Provides on-going assessment of patient response to treatments and teaching and supplies interventions as appropriate. Updates and revises plan of care as indicated.
  6. Assess and identify learning needs of client and family in order to implement effective teaching methods.
  7. Maintain complete and accurate medical records via electronic health record system and/or paper chart appropriately including visits assessment, verbal orders accurately and promptly and within the standards and initiate appropriate billing.
  8. Communicates with physicians to confirm and update patient’s plan of care. Provides status updates and ensures ongoing communication with the physician and other members of the health care team.
  9. Facilitates the discharge planning process according to standards and utilizes resources to assure continuity of care after discharge. Participates in developing the discharge plan with the patient/family, physician, social worker, and other staff members as appropriate.
  10. Participates in chart audits and continuous quality improvement activities as requested. Identifies, develops and participates in process improvement opportunities that will enhance the quality of services provided.
  11. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
  12. Maintain knowledge of organizational policies, regulations and procedures to administer patient care within the nursing scope of practice.
  13. Assess and evaluate the environment to ensure patient safety and compliance with standards of practice.
  14. Maintain licensure and certifications.
  15. May be required to satisfactorily complete an exam or other testing requirement(s) to determine skill proficiency.
  16. Must maintain an acceptable departmental attendance record.
  17. Must be reliable and prompt when reporting to work.
  18. Required to attend job related, in-service, meetings, and training to maintain professional and technical knowledge.
  19. Must adhere to tribal law and other applicable laws as well as tribal personnel policies and procedures.
  20. QUALIFICATIONS

  1. Associate or Bachelor Degree in Nursing from a program accredited by the National League of Nursing.
  2. Must hold a license to practice professional nursing in the State of Wisconsin.
  3. Minimum of one (1) year hospital experience preferred..
  4. Basic life support certification required
  5. Experience with electronic health record systems is preferred.
  6. Knowledge of professional nursing theory and practice to give and evaluate patient care.
  7. Knowledge of medical equipment and instruments to administer patient care.
  8. Skill in identifying problems and recommending solutions.
  9. Ability to assess specific age-related needs of patients.
  10. Ability to work with little or no supervision.
  11. Must be able to adhere to strict confidentiality in all matters. Knowledgeable of the Health Insurance Portability and Accountability Act (HIPAA).
  12. Excellent communication skills. Ability to communicate efficiently and effectively both verbally and in writing.
  13. Ability to establish and maintain good working relationships with individuals of varying social and cultural backgrounds.
  14. Must submit and pass a background security check to meet the Elder/Youth License Requirement as it pertains to the position and the location of the department prior to and during the course of employment with the Stockbridge-Munsee Community.
  15. Must be willing to attend all applicable training.
  16. Must pass pre-employment drug and health screening.  Must adhere to the Tribe’s Drug and Alcohol Free Workplace Policy during the course of employment.
  17. Must be eligible for coverage under the employer’s liability insurance.
  18. Must have demonstrated ability to maintain satisfactory working record in any prior or current employment.
  19. Must be able to meet physical requirements of position.
  20. Must abide by departmental and organizational safety, testing, and uniform guidelines.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

  1. This position requires a lot of sitting with occasionally stoop, kneel, crouch, and lift and/or move up to twenty-five (25) pounds.
  2. Evening and/or weekend is required. Extended hours and irregular shifts may be required.
  3. Work environment requires excellent personal hygiene due to working in close proximity to others and in an enclosed area.
  4. A Tuberculosis (TB) Screening and/or TB Skin Test may be required.
