About

The Medical Transportation Department is available to provide rides to and from medical appointments for Enrolled Community members and their family. We have one wheel chair capable vehicle and two other vehicles. This service is provided in its entirety by the Stockbridge-Munsee Tribe. This service is intended to be used as a last resort when family or friends are unavailable.

 

Transportation to appointments will be scheduled as follows:  Elderly, Disabled, All Others. The elderly will be given first priority when scheduling transportation needs.

 

We ask that you please call one week or more in advance. Rides will be scheduled to accommodate as many customers as possible.

 

A responsible adult, other than the transportation driver must accompany all children under the age of 18 except when using the service as an eligible member.

 

All children under the age of 4 must be properly restrained in an approved child safety seat. Children ages 4 thru 8 must either be restrained in an approved child safety restraint system or a safety belt. Children 9 or older and all adults must wear a safety belt as required by Wisconsin law. (Section 347.48).

 

It is the patient’s responsibility to find a certified medical transportation service which offers disabled assistance if our service is booked to capacity, however, staff will assist with the coordination of obtaining alternate transportation if needed. 

 

Wisconsin Medicaid and BadgerCare Plus Members may be eligible for NEMT. Call 1-866-907-1493 at least two business days in advance.
